2019 Spring Program Registration

Note: The 2019 Spring Program has concluded. 2020 Spring Program information and registration will be posted in January 2020. Due to overwhelming interest in our Spring Program, we suggest reserving your spot for the 2020 Spring Program.

Our 2019 Spring Program will be open to both returning Eclipse players and players new to Eclipse. The Spring Program will run from March 18 until May 17, leading into our 2019 Eclipse Tryouts. Players will attend the Spring Program with other players from their birth year. Check out the 2019 Spring Program Practice Schedule to see when and where your player would train.

The Spring Program will provide players with two practices per week (one field / one futsal) with the Eclipse coaching staff. There will be an emphasis on individual technical development during the Spring Program, although we will mix in opportunities to play as well.

Note: Per Northern California soccer policies, players who played for other local clubs during Fall 2018 should request permission from that club to participate in our Spring Program.

To Register for the 2019 Eclipse Spring Program:
1. Fill out the registration form below for your player(s).
2. Submit payment for your player using Paypal (below) or by mailing a check to East Bay Eclipse, 3376 South Lucille Lane, Lafayette, CA 94549.
3. As the Spring Program approaches, we will be in contact to confirm receipt of your registration and send any updates about the program.

Permission to Play Policy:
I, as the listed parent/guardian of the player listed on this form, give permission for my child, a minor, to participate in all activities for the East Bay Eclipse Soccer Club. I, on the behalf of myself and my player listed on this form, release and indemnify East Bay Eclipse Soccer Club, its employees and other personnel, and all owners of fields used by the East Bay Eclipse Soccer Club, as well as their employees and other personnel, against any claims, damages, or liabilities brought on as a result of the player’s participation in the East Bay Eclipse Soccer Club. I also grant my authorization and consent for East Bay Eclipse Soccer Club personnel to administer general first aid treatment for any minor injuries or illnesses experienced by the minor. If the injury or illness is life threatening or in need of emergency treatment, I authorize the personnel of East Bay Eclipse Soccer Club to summon any and all professional emergency personnel to attend to, transport, and treat the participant.

Return to Play Policy:
If an injury sustained is or may be major, the coach should not allow the player to continue playing and should contact the player’s parents and the director. Any potential head injury is to be considered a potential major injury, and the player should be removed immediately from the field while the player’s parents are contacted. The coach and/or director can perform a concussion test to help inform the parents of the likelihood of a concussion; however, the player may not return to play until they have been evaluated by a licensed health care provider and received a clearance to return to play in writing. Coaches may not allow a player to return to play after a major injury or head injury until they receive a copy of the doctor’s clearance.
